A recent Orange Box update has apparently caused a lot of issues with Garry's Mod servers. Below is detailed information on the Update and a fix for Dedicated Server Users.
Just for further confirmation garry's mod has not been hacked like some people think due to the fake server spam.
The Update:
Orange Box Engine Update Released

Updates to the Orange Box Engine have been released. The updates will be applied automatically when your Steam client is restarted. The specific changes include:
Orange Box Engine
Fixed stats and achievements not working in Half-Life 2: Episode Two and Portal
Fixed incompatibilities with MODs
A temporary fix has been reported by haza55!
Download GCF Scape
Go into your SteamApps directory.
Open "Source 2007 Binaries.gcf"
Extract the contents of the "Bin" directory into your servers "Orangebox/bin" directory.
Start your server and enjoy.
